Medical Escort
Accompany follow-up appointments, assisting with registration, medication collection and care along the way, and report back to the family as arranged.
Health Care Worker (HCW)
Take and record basic vital signs, carry out designated activities and daily care, as set out in the care plan.
Personal Care Worker
Assist with daily living, companionship, light housekeeping and medication reminders on the agreed schedule.
Physiotherapist
Home assessment and training in support of mobility, balance and walking needs.
Occupational Therapist
Assess daily living function and the home environment, and advise on activity and care adjustments.
Speech Therapist
Assess communication and swallowing needs, providing training and family guidance as appropriate.
Dietitian
Assess nutrition and dietary needs, and provide individualised recommendations.
