In-home Health Assessment

Care Starts WithOne Clear Assessment

When an older adult has multiple concerns at once — or returns home after a hospital stay in a more complex condition — the hardest part is often knowing what to address first. CareLane sends a professional to assess the full situation at home, identify the key risks, and give you a clear care direction.

Who this service is for

When is an assessment most helpful?

If you are unsure whether to arrange a carer, nurse, or therapist first — or your loved one has multiple issues at once — a full picture first is almost always easier than guessing one piece at a time.

Unsure which service to arrange

Not clear whether to start with a carer, nurse, or therapist.

Health condition has changed

Reduced mobility, a fall, swallowing difficulty, or noticeable weight change.

Complex discharge from hospital

Multiple issues at once — needs a fresh plan for home care after leaving hospital.

Several problems at the same time

Cognition, wounds, nutrition, transfers, and home safety all overlap.

Assessment scope

One visit, six areas reviewed together

We do not focus on one problem only. We break down the older adult's home situation into six areas — so the care direction reflects what is actually needed.

01

Mobility

Standing, walking, transfers, toileting, and getting in and out of bed — with fall risk assessed.

02

Daily Self-care

Eating, dressing, and bathing — to understand how much assistance is needed day to day.

03

Cognition · Communication · Mood

Memory, expression, understanding, and mood — watching for signs of decline or low spirits.

04

Swallowing & Nutrition

Chewing, swallowing, meal portions, and nutrition risk — to reduce choking and weight loss.

05

Skin & Nursing Needs

Skin condition, wounds, pressure sore risk, and whether specialist nursing follow-up is needed.

06

Home Environment Safety

Fall hazards and care difficulties at home — with practical, actionable improvement suggestions.

Assessment Report and Care Plan

After the assessment, you get a clear set of recommendations

The assessment does not end at "we came and looked." We organise findings into a concise written summary — giving you and your family a clear basis for decision-making, communicating with clinicians, and following up later.

  • Key risks — The most important concerns identified from the assessment.
  • Priorities — What to address first, and what can follow later.
  • Service direction — A suggested mix of care, nursing, or therapy.
What you get

Not just a visit — a clear starting point

After the assessment, you will have a much clearer grasp of the situation and know where to go next.

Clearer picture

Scattered worries are organised into an understandable, concrete picture — no more guessing.

Priorities set

Know what needs handling urgently and what can follow later — feel more in control.

Suitable care direction

A suggested service mix: home care, nursing, or therapy — based on real findings.

Ready for next steps

Know what to prepare before arranging formal care — no unnecessary detours.

✦ CareLane's approach

A good assessment sees clearly and explains concretely

A good assessment is not just a visit. It turns a complex situation into recommendations the family can actually use.

01 · Holistic review

Assess everything together

From mobility and self-care to cognition, nutrition, and home safety — reviewed in one visit for a complete picture.

02 · Professional judgement

Nursing team assessment

Professionals assess and coordinate — judging severity and priority to give you a well-ordered set of next steps.

03 · Clear handover

Written summary to act on

A concise written summary makes family discussion, clinical coordination, and future follow-up straightforward.

How it works

From a brief description to a clear recommendation

1
Describe the situation

WhatsApp or call us with a brief description of the older adult's situation, particularly any recent changes or hospital discharge. Enquiries are followed up during business hours, subject to current availability.

2
Schedule the home visit

We arrange a time convenient for you and your loved one, and explain in advance what the assessment covers and approximately how long it takes.

3
Professional assessment at home

Our professional reviews six areas on-site: mobility, self-care, cognition, swallowing and nutrition, skin care, and home safety — in the actual home environment.

4
Written advice and direction

We compile key risks, priorities, and a recommended service mix into a clear written summary — a concrete starting point for what comes next.

Common questions

What families ask most

Do I have to start a service right after the assessment?
Not at all. The assessment is about seeing the situation clearly. You can take the recommendations and decide at your own pace — or simply complete the assessment without arranging anything further.
Which older adults benefit most from an assessment?
Those with recent health changes, recovering from a hospital stay, or where the family feels overwhelmed and unsure where to begin — all benefit most from getting a full picture first.
Is an assessment only for serious cases?
No. Families who want to get care arrangements right early — before problems escalate — benefit just as much. Earlier understanding is almost always easier to act on than catching up after a crisis.
Can I arrange follow-up services based on the assessment?
Yes. If you decide to arrange home care, nursing, or therapy afterwards, the assessment findings feed directly into the plan — no need to explain the situation all over again.
How is the assessment fee calculated?
It is a one-off fee based on scope and visit arrangements. If follow-up services are arranged, the assessment can be integrated into the overall package. WhatsApp us for an initial quote.
