守護里長者服務有限公司 CareLane Elderly Service Limited (the "Company") respects and is committed to protecting your personal data privacy in compliance with the Personal Data (Privacy) Ordinance (Cap. 486). Personal data collected in this form (including identity documents, professional certificates and payment account details) will only be used for processing your application, verifying qualifications, arranging service cases and paying service fees. Data is stored encrypted and accessible only by authorised staff.
Submitting this application does not guarantee that the Company will engage you. The Company will screen applications according to service needs and applicant qualifications, and reserves the sole discretion whether to invite cooperation; the Company is not obliged to give reasons if an application is not accepted. Your application data will be retained for up to 24 months so that we may contact you when suitable service cases arise; you may request access, correction or deletion of your data at any time by contacting info@carelanehk.com.
Independent Contractor Agreement — Terms & Conditions (Summary)

- The Company is a home care service provider contracting directly with clients. The Company is NOT an employment agency and does not provide job placement or labour supply services, nor does it charge contractors any placement fee or commission.
- This is an on-demand independent contractor service agreement and does not create an employer–employee relationship; tax, MPF and insurance responsibilities rest with the contractor.
- Contractors are free to accept or decline service cases and must hold valid practising certificates and personal liability insurance.
- Service fees are settled monthly and paid on the 14th of the following month via your chosen payment method (bank transfer / FPS / cash cheque); FPS and cash cheque payments are made payable to the name on your HKID.
- Contractors must observe confidentiality obligations protecting client health information and Company confidential information.
- Submitting this application does not guarantee engagement; the Company screens applications according to service needs and qualifications and reserves the final decision. Application data is retained for up to 24 months for future contact.
