Senior Home Care

Stay HomeKeep the Same Life

CareLane arranges daily living support, personal care, feeding assistance, mobility help, companionship and medication reminders based on how much the older adult can still manage. A Registered Nurse assesses first, then we agree on hours, staffing and exactly what each visit covers.

When Home Care Helps

Whether your loved one is recovering from surgery, managing a chronic condition, or at risk of falls when left alone — CareLane's home care is designed around the senior's actual needs, not a generic package. We listen first, assess when needed, and build a care plan that works for the whole family.

Who is it for

Who Is This For?

Home care suits a wide range of situations — from short-term recovery to long-term daily support.

1

After Hospital Discharge

Recovering at home after surgery, hospitalisation, or a fall? We provide structured short-term support with health observation to help seniors transition safely back home.

2

Limited Mobility

Difficulty with transfers, bathing, dressing, or indoor walking? We assist with all personal care and mobility tasks to keep seniors safe, comfortable, and independent daily.

3

Living Alone

Seniors living alone face higher risks from falls, missed meals, and medication gaps. Regular home visits bring daily structure, companionship, and a reliable safety net.

4

Caregiver Respite

Family caregivers need rest too. Scheduled respite care reduces burnout, gives you breathing space, and allows you to manage other responsibilities without guilt or worry.

5

Chronic Conditions

Conditions like diabetes, COPD, heart disease, or post-stroke require consistent daily observation. Our caregivers help with medication reminders, health monitoring, and safe activity support.

6

Night-time Risks

Night toileting, wandering, confusion, and falls are common risks for frail elderly. Night care provides support through the hours when family members cannot always be awake.

03 · Companionship

Emotional Support & Cognitive Activity

Loneliness affects both mental and physical health in elderly. Our caregivers provide patient conversation, reading, walks, and simple cognitive activities — building trust, routine, and emotional wellbeing.

  • Patient conversation, reading, and reminiscing together
  • Gentle exercise, outdoor walks, and light cognitive tasks
  • Building familiarity and trust for a calmer, happier senior
Daily rhythm

A Typical Day of Home Care

Each senior's daily care is different. This is a common reference for a daytime care arrangement.

Morning Routine

Help the senior get up, wash, dress, and complete oral hygiene. Prepare and assist with breakfast, give medication reminders, and note overnight condition and mood.

Activity & Health Check

Accompany a gentle walk or light stretching. Check blood pressure or other indicators as required. Record mood, appetite, and any concerns for the shift note.

Midday Rest & Safety

Support lunch preparation and eating, assist with toileting, repositioning to prevent pressure sores, and a quick home-safety check for any new hazards.

Handover to Family

Prepare a clear handover record covering all care provided, health observations, medication taken, and anything the family needs to follow up or be aware of.

Why CareLane

Why Families Choose CareLane

Nursing-led coordination

A qualified nursing team designs and reviews your care plan — not just assigns staff. This ensures clinical safety, consistency, and a plan that adapts as needs change.

Thoughtful caregiver matching

We match by personality, language preference (Cantonese, Mandarin, or English), gender, and clinical experience — because trust between caregiver and senior matters deeply.

Transparent shift records

After every visit, a clear handover note is prepared so family members know exactly what was done, what was observed, and what needs follow-up. No guesswork.

Flexible, no rigid contracts

As health needs change, we review and update hours, scope, and staffing accordingly. Start short-term and extend if needed — on your terms.

Start care

How to Get Started

We listen to the situation first, assess when needed, then match the right caregiver and follow up as care evolves.

01

Initial Enquiry

Tell us the senior's age, main health conditions, care needs, district, and preferred timing. We provide initial guidance during business hours, subject to availability.

02

Home Assessment

For more complex situations, we arrange a nurse to visit at home — assessing mobility, health risks, cognitive state, and the environment before designing the care plan.

03

Caregiver Match & Confirmation

We confirm a caregiver matched to the senior's needs, language, gender preference, and personality — and agree on the schedule and quote with the family.

04

Ongoing Review

The nursing team reviews care regularly. When conditions change, we review requested changes and confirm any revised plan, hours or staffing subject to care needs and availability.

FAQ

Frequently Asked Questions

Can caregivers perform medical procedures?

Personal Care Workers (PCWs) handle daily living tasks. Clinical procedures are assigned to an appropriately registered nurse who is trained and competent for the procedure, within professional scope and according to medical instructions. The scope of each role is clearly defined in the care plan from the start.

Can we request a gender or language preference?

Yes. We take gender preference, language (Cantonese, Mandarin, or English), and personality compatibility seriously in every caregiver match. Please let us know your preferences and we will do our best to accommodate them.

What is the minimum booking length?

Booking length depends on the service type, district, staffing and care needs. The confirmed minimum and fees will be explained before service begins.

Can the Community Care Service Voucher (CCSV) be used?

CareLane works with recognised service providers under the CCSV scheme. Actual voucher use depends on SWD approval, provider scope and confirmation of the applicable service. CareLane does not present itself as a recognised service provider.

How soon can service start?

Start time depends on the service type, district, time slot, staff availability and whether an assessment is needed. We confirm availability before making any commitment.

What if the assigned caregiver is not a good fit?

We ask for your feedback after the first few visits. If the match is not working well, we reassign. Our goal is a stable, trusted relationship between the caregiver and your loved one — and we take that seriously.
